  • Background: Chronic kidney disease is associated with multiple comorbidities and a high cost for the health system. Kidney transplantation is the definitive treatment option. There is a significant gap between donor supply and recipient demand, which is why the use of expanded criteria donor organs (ECD) is a viable therapeutic option.Methodology: Historical cohort analytical observational study.Results: The analysis included 1047 patients, 20.24% (n = 212) are part of the group of transplants with ECD. Lower graft survival was evidenced in patients transplanted with this type of organs; by the third year after transplantation, patients with a donor with standard criteria have a graft survival of 91% (CI 0.8851 - 0.9320), in patients with ECD it falls to 78% (0.69 - 0.84). At the fifth year post transplantation, a similar trend is evidenced with a survival of 85% for the first group vs 67% for the SCD (CI 0.82 - 0.88 - CI 0.55 - 0.77). A statistically significant association was evidenced for graft loss in the patients of the latter group (HR 6.6; 95% CI 2.4-18.3).Conclusion: Transplant patients with ECD have a lower survival compared to those who received a standard criteria graft, however, it must be taken into account that the use of ECD benefits patients, improving their survival and quality of life when compared with those who remain on dialysis
