abstract

  • Introduction: Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation, gradually and almost irreversible, associated with the inflammatory reaction attributed to several factors, mainly the snuff smoke exposure. It is considered a public health problem in Colombia and in the world, with a rapid increase in the chronic condition today. Objective: To identify sociodemographic differences, clinical and treatment among patients with clinical and spirometric COPD patients vs clinical diagnosis and discarded by spirometry Suba Hospital. Material and Methods: An observational, descriptive, retrospective and exploratory component to compare groups with clinical diagnosis of COPD confirmed or ruled by spirometry, between January and August 2011. Descriptive statistics were used to calculate the measures of central tendency, quantitative data were expressed as mean ± standard deviation variable, and the qualitative and percentage, the Student t test to analyze the difference between the means of quantitative variables and groups Pearson test to analyze the relationship between the qualitative data for those with expected values below 5 was applied Fisher's exact test, we consider a α of 0.05 for the bivariate analysis and measures of association. All analyzes were performed using SPSS version 19.0 Corporate. Results: Of the 398 patients, only 287 met inclusion criteria. The average age of all patients was 70.29 + 11.18 years, and 59.5% of the population was female. Of the patients evaluated, 171 patients (59.6%) was ruled out the diagnosis of COPD (FEV1 / VEC> 0.70). When comparing groups of patients who had confirmed the diagnosis of COPD by spirometry cons the discarded found no statistically significant differences between age, in patients with COPD female predominance (p 0.02), on the factors risk exists clear association between COPD and exposure to wood smoke (p <0.001), and as for the smoking association was found only with an ex-smoker (p 0.011). To analyze the differences in treatment was stratified by the possible combinations with or without theophylline inhalers, a statistically significant finding treatments for three inhalers (p 0.015), two inhalers + theophylline (p 0.05), three inhalers + theophylline (p <0.001), and in untreated patients (p <0.001).
