  • Background: The Antimicrobial Stewardship Programs are new strategies that have been created to improve the use of antibiotics. In the neonatal unit of Fundación Cardioinfantil the program was implemented since 2018. However, since its beginning, have not been created studies that allow knowing its results. It is proposed to evaluate the impact in a high-complexity neonatal unit in Bogotá. Methodology: Is an observational descriptive study with an analytical component, where the clinical history of the patients was collected and for the analysis of the results used hypothesis test t-student or U-Mann Whitney for the quantitative variables and the independency Chi-quad test for the qualitative variables. Results: It found deviations compared with international literature, mainly a major use of antibiotics after the implementation of the antibiotics program, but guided according by positivity and colonization screening. It was a decrease in the prevalence of germs such as S. aureus, E. coli, E.faecalis, and outcomes such as the presentation of necrotizing enterocolitis and septic shock. Conclusion: Antimicrobial optimization programs generate a positive impact in neonatal units with concerning guidance and use of antibiotics, microbial resistance, and positive outcomes for the neonatal unit and newborns. This can generate an impact on costs associated with health care.
