  • Introduction: The management of penetrating abdominal injuries has changed over the past 30 years, mandatory laparotomy behavior is increasingly used in the Kennedy Hospital despite complications and increased mortality rates. There are different selective behaviors such as open abdominal trauma: serial physical evaluations, ultrasound, laparoscopy, triple-contrast CT and DPL, but not defined which of these methods is most efficient for the diagnosis. The objective is to make a sistematic review of the literature so we can make a clinical guideline. Methodology: Systematic review of the literature in indexed publications and email addresses of storage entities of clinical guidelines. Were selected articles published between 1990 and 2010 to provide the best evidence. Methodological quality was assessed according to the recommendations developed by the American Medical Association JAMA 1993. Additionally we used the methodological quality scores published in the journal Surgery Chilena 2003. Included studies were organized in a table of evidence which took into account the following points: date of publication, journal, author, level of evidence and main outcome. Results: In total 98 articles were found, according to the eligibility criteria were selected 57 and was added 2 clinical management guidelines. Conclusion: There is a trial to determine which diagnostic method is the most efficient in the conservative management, but if it is clear that the current trend of management of abdominal trauma is directed to select and focus on the individual patients.
