  • The project is a proposal to solve a business problem that originates in social, economic and cultural changes worldwide, which have impacted on demographic dynamics, which has led to an increase in the life expectancy of the population and consequently Due to an increase in the prevalence of chronic non-communicable diseases, which demand greater resources from the health system, some patients in the hospital setting may need admission to out-of-hospital special care programs, some of them already known as the Plans of Home Hospitalization, Chronic Care Units, Palliative Care Units, Pain Clinic, etc., for the management of specific clinical conditions that do not require hospitalization, or that by achieving control of the patient's condition it is not necessary to continue it in the hospital, but they do require monitoring and surveillance of acute symptoms and treatments that should be continued nuar, which can be done at home or in places established, appropriate and authorized by the insurance companies and independent IPS, through the previously mentioned programs, however, access to these possibilities at the most appropriate time for the patient, many times it is limited mainly by inopportunity in the response by the insurer or provider, the non-coverage by the provider in the patient's area of residence, the non-availability of home hospitalization plans by the insurer and other family-type conditions, additionally, in cases where intrahospital management has been initiated, on many occasions these generate prolonged stays that increase the risk of complications, higher costs for the health system and lower efficiency of the institution by reducing the availability of beds in this way. By knowing this problem, with the design of the intramural hospital extension program at the Palermo Clinic, the causes of inopportune access to the aforementioned special programs could be mitigated, provide continuity of care, improve the safety and satisfaction of the patient and family, and with the release of beds, improve the efficiency of the institution and reduce costs to the General System of Social Security in Health.
