- ObjetivoEvaluar, en una cohorte de pacientes con artritis reumatoide (AR) tratados con fármacos subcutáneos contra el factor de necrosis tumoral (anti-TNFs), los niveles de adherencia al tratamiento antes y después de implementar un modelo de atención integral (MIA).Pacientes y métodosSe realizó un estudio observacional que incluyó pacientes con AR en tratamiento con anti-TNFs subcutáneos (adalimumab, etanercept y golimumab) seleccionados a conveniencia; se calculó un tamaño muestral de 125 pacientes. La variable de resultado fue la adherencia evaluada con el Cuestionario de Cumplimiento en Reumatología (CQR19), medida antes y después de implantar un MCP. Se realizaron análisis descriptivos y bivariantes comparando la adherencia antes y después de aplicar el modelo (prueba de Wilcoxon y Chi2 de McNemar). Para el análisis multivariante, se realizó un modelo lineal generalizado ajustado por covariables, donde la diferencia en la proporción de adherencia fue la medida de resultado.ResultadosUn total de 131 pacientes con AR fueron seguidos durante 24 meses; la edad media era de 62 años, y el 83,9% eran mujeres. La mediana de la DAS28 al inicio del seguimiento fue de 2,32, y la del HAQ, de 0,25. Al inicio, el 87,8% eran cumplidores; después de 24 meses, el 96,2% eran cumplidores según el CQR19. Al final del seguimiento, la adherencia aumentó con los tres tipos de tratamiento anti-TNFs. En un modelo emparejado ajustado por variables clínicas, se estimó que la MCP producía un aumento del 9,4% en el porcentaje total de pacientes adherentes. Además, se encontró un aumento estadísticamente significativo del 4,5% en el porcentaje de pacientes adherentes tratados con golimumab en comparación con etanercept y adalimumab.ConclusiónLa MCP produjo un aumento importante del porcentaje de pacientes con artritis reumatoide adherentes al tratamiento tras 24 meses de seguimiento. Cabe destacar que los pacientes con golimumab fueron más adherentes en comparación con otros tratamientos anti-TNF actuales.

The outcome variable was adherence assessed with the Compliance Questionnaire on Rheumatology (CQR19), measured before and after implementing a CCM. Descriptive and bivariate analyses were performed comparing adherence before and after applying the model (Wilcoxon and McNemar’s Chi2 test). For multivariate analysis, a generalized linear model adjusted for covariates was performed, where the difference in the proportion of adherence was the outcome measure.ResultsA total of 131 RA patients were followed-up for 24 months; average age was 62 years, and 83.9% were women. The median of DAS28 at the beginning of the follow-up was 2.32, and the HAQ was 0.25. At baseline, 87.8% were adherent; after 24 months, 96.2% were adherent according to CQR19. At the end of follow-up, adherence increased with the three types of anti-TNFs treatment. In a matched model adjusted for clinical variables, the CCM was estimated to produce a 9.4% increase in the total percentage of adherent patients. Additionally, a statistically significant increase of 4.5% in the percentage of adherent patients treated with golimumab compared with etanercept and adalimumab was found.ConclusionA CCM produced an important increase in the percentage of patients with rheumatoid arthritis adherent to treatment after 24 months of follow-up. It is noteworthy that Golimumab patients were more adherent when compared with other current anti-TNFs treatments.
