  • The purpose of this work is to review the medical literature related to the reconstruction of the amputated hand, specifically in the so-called metacarpal hand and present one clinical case of a patient who underwent the transplantation of three of his toes to restore the tripod clamp in his affected hand, procedures performed at the Clínica del Occidente in Bogotá. The reconstruction of the dominant right hand in a metacarpal hand type I is described, secondary to the surgical amputation derived from a hot asphalt burn, performed in two sequential microsurgical procedures, as well as the degree of functional recovery thereof in the late postoperative period. . The DASH scale (Disabilities of the Arm, Shoulder and Hand ) validated for the Spanish language it was applied to assess the degree of pre- and post-operative disability. The clinical evaluation of the patient showed the development of a competent, sensitive tripod clamp, with sensitive discrimination, which was correlated with a 60-point increase in the DASH scale, from pre to postoperative. It is concluded that the reconstruction of the metacarpal hand with autologous toes transplantation is a reproducible technique in our environment (without neglecting that it requires a high level of training by the group of treating surgeons), which offers a functional and aesthetic result that It allows the patient to regain their autonomy in relation to the basic functions of their hand.
