  • Carpal perilunate injuries are complex and unusual injuries that can be overlooked in the initial assessment of the patient, which can lead to early osteoarthritis, chronic instability of the carpus, nonunion of fractures. Surgical management is the one that has been established as the one that has the best functional result for the patient, hence the importance of identifying these injuries. Depending on the structures affected, there may be injuries to the minor carpal arch where there is only capsuloligamentary compromise or injuries to the major carpal arch where, in addition to the ligamentous part, there are fractures of different carpal bones, depending on the propagation of the force of the trauma. Various patterns of involvement of the greater carpal arch have been identified and those of palmar fracture dislocation are the rarest. This case report shows one of these unusual patterns with its surgical management and functional outcome
